    I have worked for 20 years in the Agile Industrial complex and as a community, we have focused way too much on the build side of the house and not enough on the business side.  That is increasingly changing to a model of Strategic Execution and focusing on the value first and then optimizing delivery with Agile.  

    Many organizations are adopting End 2 End Value Stream Teams (E2E VSTs) and redesigning their organization's structure, funding and governance to support these value delivery engines.  These organizations are still a minority but the rate of adoption of this model seems to be at a tipping point. This involves engaging in a critical discussion of what is value and constantly prioritizing value first.  To do this many, people are aligning E2E VSTs with product lines or lines of business.  To truly go E2E many of them are embedding business functions with delivery teams.  These functions such as business operations, marketing, HR, legal, planning, funding and of course the folks that define what the right thing to build is.  By cutting across all those silos to create delivery teams we allow things to go from *Idea to Implementation* or *Concept to Cash* very quickly.

    This idea is not new and there are many examples and names for these types of teams.  It was embodied in the Toyota Production System over 75 years ago, adopted by product companies and is just the way many startups work.  Focus on value first, determine how to get it to flow through the system, engage continuous improvement to improve the product and the process.  Then structure (and restructure) the organization to optimize the flow of value in a sustainable way understanding that everything else the organization does is secondary to the flow of value.

    This talk will focus on giving perspective on these basic questions regarding End 2 End Value Stream Teams.

    This presentation is designed to show the many different variables that lead to clinician dissatisfaction with problem list interactions. These variables lead to poor reporting, inaccurate diagnosing and incomplete or insufficient reimbursement and clinician burnout.

    With a more managed approach to the problem list. The clinician can better diagnose and can capture the true clinical nature of the patient problem and the true clinical intent of the provider. It can also eliminate lapsed problems that remain on the list, duplicate problems and missing terms needed for a more granular diagnosis.

    Diligent problem list management can increase provider satisfaction, provide for more accurate problem diagnosis and increased patient safety. It can lead to better and more defined reimbursement and save time and effort on behalf of the provided freeing them up to do other important activities.
